Researchers and doctors at the North Shore-LIJ Health System and the Feinstein Institute for Medical Research have discovered a potential explanation for why breast cancer is not experienced the same way with African American and Caucasian patients. This data will be presented at the 2012 American Society of Clinical Oncology (ASCO) Annual Meeting to be held from Friday through Tuesday (June 1-5) in Chicago, IL.

Breast cancer is more common in Caucasian women than in African American women; however, African American women experience a more aggressive form of breast cancer that occurs almost a decade earlier than Caucasian women. Because of this, African American women have a lower breast cancer survival rate than Caucasian women. To explore the reasons why, researchers and doctors at the North Shore-LIJ Health System and the Feinstein Institute for Medical Research conducted a study to determine 1) why the expression of a genetic marker embedded in deoxyribonucleic acid (DNA), called microRNA, differs between African American and Caucasian women, and 2) if variation in microRNAs may explain the observed survival difference between African American and Caucasian women.

In this study, microRNA profiles from the blood of 32 female patients were collected before removal of breast tumors. The mean age of the patients was 50 years, ranging from age 31 to 68, and 10 of the patients had stage III triple-negative breast cancer (five were African American and five were Caucasian), 10 patients had stage III estrogen-receptor or progesterone-receptor positive breast cancer (five were African American and five were Caucasian), and 12 patients were controls (six were African American and six were Caucasian). Triple-negative breast cancer refers to any breast cancer that does not express three receptors known to advance most breast cancers; estrogen receptors, progesterone receptors and human epidermal growth factor receptor 2 (HER2). Although triple-negative breast cancer is estrogen-receptor negative, progesterone-receptor negative and HER2 negative, and the most successful treatments for breast cancer target these receptors, triple-negative breast cancer typically responds to chemotherapy.

The study found that, 1) female Caucasian patients who had triple-negative breast cancer overexpressed 20 microRNAs (15 times higher than the controls), and none of the microRNAs these patients had were found in any of the African American patients, 2) female African American breast cancer patients overexpressed only six microRNAs (15 times higher than the controls), and none of these microRNAs were detected in Caucasian patients who had triple-negative breast cancer, and 3) four microRNAs in African American patients and eight microRNAs in Caucasian patients were not previously reported in association with breast cancer, which suggests that they may be connected to how the patient reacts to cancer.

"The striking difference in the patterns of microRNA expression between African American and Caucasian breast cancer patients may provide insight into answering why, when receiving similar treatments, outcomes are different between African Americans and Caucasians," said Iuliana Shapira, MD, director of the Cancer Genetics Program at the North Shore-LIJ Health System's Monter Cancer Center. "Breast cancer patients who have the most devastating outcome may carry the microRNAs that promote cancer. What we saw in this study is that Caucasian women may carry microRNAs that protect against cancer while African American women do not express those microRNAs. The lack of expressing these microRNAs in African Americans could be the cause of poor outcomes seen in these women. Methods to increase microRNAs in the blood before surgery for cancer, such as giving chemotherapy before surgery for cancer, may improve survival rates in African American women with triple-negative breast cancer."

verizon Communications will introduce new high-speed Internet service tiers and pricing next month, in one case doubling its fastest level of service to 300 Megabits per second.

The new FiOS Internet portfolio will feature download/upload speeds of 50/25 Mbps and 150/65 Mbps and two new tiers of 75/35 Mbps and 300/65 Mbps.

verizon will continue to offer its entry level speed of 15/5 Mbps as well as 25/25 Mbps, 35/35 Mbps, 50/20 Mbps and 150/35 Mbps.

Pricing for the new portfolio will be announced next month. Current FiOS Internet customers will be able to maintain their existing tiers of service.

The new speeds will be offered in standalone and bundled packages and are designed to address the growth of bandwidth-intensive applications and the increase in the number of Internet-connected devices being used simultaneously in the same household.

In a statement, Verizon said the new speeds will also support consumers who are watching more over-the-top video programming on TVs and portable devices, and accommodate the rise in Internet-enabled applications like video and audio streaming, home monitoring devices, video chat, multiplayer gaming and online backup services.
?The ways we used the Internet and watched TV over the past 10 to 15 years have dramatically shifted,? said Verizon?s consumer and mass market business unit president Bob Mudge in a statement. ?With the emergence of smartphones, smart TVs, Blu-ray players, tablets and gaming consoles that also serve as over-the-top devices, consumers need more bandwidth to receive the highest-quality experience.?

In a statement, Verizon said that the 75/35 Mbps service is geared toward households that download HD movies, participate in multiplayer gaming and have three or more interconnected users. The highest tiers (150 and 300 Mbps) are targeted at households with five or more interconnected users who want to download high-quality HD video on multiple devices.

For example, with a 300 Mbps connection, consumers can download a two-hour, standard-definition movie (1.5 Gigabytes) in less than 40 seconds; and a two-hour, high-definition movie (5 GB) in 2.2 minutes. On a 150 Mbps connection, the same two-hour, SD movie can be downloaded in less than 80 seconds, and the two-hour, HD movie in less than 4.5 minutes.

?High-speed Internet no longer is just for techies, as more than half of our residential consumers already use at least a 20 Mbps Internet connection,? Verizon?s consumer and mass market business unit chief marketing officer Mike Ritter said in a statement. ?Streaming online video on an all-fiber-optic connection providing faster speeds is better and more reliable during peak Internet usage hours.

?As recently as 2005, video was less than 10% of Web traffic,? said Ritter. ?By the end of this year, we expect it to be 50%, growing to 90% in just a few years.?

Verizon estimated that today, the average home has seven Internet-connected devices, growing to between 9 and 15 devices by 2015.

The top two tiers require customers to be connected via a GPON (gigabit passive optical network) terminal. For existing customers who have a BPON (broadband passive optical network) terminal and qualify for the GPON service, the installation of a new GPON terminal will be required.

Book Review: They Shoot Horses, Don't They? by Horace McCoy ...

Horace McCoy, along with James M. Cain and a few other authors of the '20s, '30s & '40s, was labeled early on as a hardboiled author, in the same vein as Raymond Chandler, Dashiell Hammett and others. But, in retrospect, most of his novels didn't fit the classic description of hardboiled.

First, they weren't detective novels. Secondly, the main protagonists were not mainly dealing with solving a mystery and often the main characters were flawed, and if not totally beyond redemption, at least of questionable character. Of course, now these writers are most identified with the 'Noir' genre; the 'roman noir' or "dark books' as they would finally be labeled after many of them were made into film noirs of the '40s and '50s.

Reissued in April by Open Road Media, They Shoot Horses, Don't They?
is McCoy's best-known novel, perhaps because of the Academy Award-nominated film made from it (it won one Oscar for Gig Young as supporting actor, and was nominated in eight other categories, including best director- Sydney Pollack and best actress - Jane Fonda) starring Michael Sarrazin and Jane Fonda.

The story opens up with the narrator and protagonist confessing to murder, thus quickly shedding any aspirations to a mystery. He confesses that he "killed her," and that he doesn't "have a leg to stand on" alluding to the title of the book. In his youth, he saw the favorite family horse break its leg, after which it was shot and put out of its misery, also leading to the conclusion as well as the title.

The main characters are Robert Syverten, who came to Hollywood with dreams of becoming a great director, and Gloria Beatty who ran away to Dallas from a farm in West Texas where her uncle always made passes at her. In Dallas, she tried to commit suicide, then ran away to Hollywood with dreams of being in movies, but is finding only rejection. The pair meet on the morning when they have both failed to get parts as extras.

The setting for the novel is in the shabby La Monica Ballroom, perched over the Pacific Ocean on the Santa Monica Pier, near Los Angeles. Gloria talks Robert into participating and entering a marathon dance contest, believing the contest may be a way to get noticed by studio producers or movie stars. The novel was written in 1935, at the height of the dance marathon craze. Dance endurance contests attracted people to compete to achieve a type of cheap celebrity or monetary prizes. McCoy, whose life story very much resembled Roberts, had briefly worked as a bouncer at contest just like this in California.

The book also differs from the majority of hardboiled books of the era in that the plot is nonlinear, being told back to front, with additional flash-back scenes that reveal the characters' back story and histories. From the start we know that Robert killed Gloria and the plot reveals why.

Further, the flashback scenes show Robert witnessing the shooting of the family's pet horse and how that affected him, his hopes and dreams of becoming a great film director, and the failure he feels in just hanging on and accepting bit parts. There's also Gloria's life with the lecherous uncle, her despair in trying to make it as a single girl at the height of the depression and why she tried to kill herself in Dallas before coming to Hollywood to become a movie star.Hardly classic hardboiled.

All of this reveals why Gloria, from the start has a fatal outlook for her future, and maybe everyone's future. From the start, Gloria tells Robert that she wishes she were dead, a point she repeats in most of their conversations. She also feels she has had to resort to unsavory acts, one of which will come out later in the dance contest, just to survive. One hundred and forty four couples start the contest. Robert and Gloria, like most of the contestants, are young, jobless, and drawn as much by the free food as by the $1,000 prize money (raised to the 'princely sum of $1,500 in the movie).

As the book begins, Robert is advised to beg for mercy from the court. The story of his relationship with the Gloria is then intercut after every few chapters with short excerpts from the judge's sentencing. The excerpts of the judge's words are written in larger and larger type until the last page of the book concludes with the words written in small print: "And may God have mercy on your soul".

They Shoot Horses, Don't They? has long been considered, along with The Grapes of Wrath as one of the most convincing - and heartbreaking- fictional portraits of America during the Great Depression. It is a stunning portrait of poverty and powerlessness during the Great Depression. As the dance goes on, into the second and third week, the crowds grow larger and the media come out, as perversely hoped for by the contestants, to cover and sensationalize the contest.

Also, the rich and famous who have made it, come out to watch, almost like people stopping to look at an auto accident. As the contest grinds on, couples break down physically and drop out, and some plot desperately to gain an advantage. The sponsors also stoop to new lows in the hopes of profiting from the desperate dancers. There are rumors of the contest being fixed half way through, and then they weather an attempt to shut down the contest by the local morals society. Gloria, angry, bitter and outspoken in her desperation, curses the women as spoiled, interfering hypocrites.

I believe that what makes the novel a classic isn't just its depiction of life in The Great Depression, but its larger portrail of life in every generation. It reveals how easy it is for dreams to crumble, and also how our egos can work against us by giving us goals that are beyond our capability to achieve. Gloria dreams and searches for redemption by becoming a Hollywood starlet, when in reality she is not that attractive. This universal theme is what makes the novel a classic and not just a snapshot of a desperate time in history.

It's wonderful to see the book republished by Open Road, who released this book in eBook at the same time they republished McCoy's other masterpiece of classic noir, Kiss Tomorrow Goodbye. Both eBooks feature an extended biography of Horace McCoy and are nicely formatted.

Why is it so difficult to trace the origins of food poisoning outbreaks?

As illustrated by the 2011 E. coli outbreak in Germany in 2011, any delay in identifying the source of food poisoning outbreaks can cost lives and cause considerable political and economical damage. An international multidisciplinary team of scientists have shown that difficulties in finding the sources of contamination behind food poisoning cases are inevitable due to the increasing complexity of a global food traffic network where food products are constantly crossing country borders, generating a worldwide network.

As consumers we are used to seeing country of origin labels on certain foods, but what about on products with more than one ingredient? A recent study by the Food Safety Authority of Ireland showed that 53 countries contributed to the ingredients of an ordinary "Chicken Kiev" in a Dublin restaurant. This diversity of sources is partly to blame for the failure to identify the sources of food poisoning outbreaks, and has lead to calls for international health agencies to initiate a system to monitor this 'human food web.' But just how complex is the human food web? What is its structure, can we quantify it, and what can we learn from it?

In the first study of its kind, published in the journal PLoS ONE, the scientists studied databases of food import and export to understand how 'food fluxes' generate a complicated worldwide network. They were led by Professor J?zsef Baranyi of the Institute of Food Research, which is strategically funded by the Biotechnology and Biological Sciences Research Council.

Using agri-food import-export data from the UN and FAO databases, the authors chart out the worldwide food-transport network and show that it forms an amazingly complex transport web. With the help of network science methods they reveal that it has highly vulnerable hotspots and demonstrate that, without increased control, some of these are prime positions for making outbreak tracing difficult.

The research identifies a number of countries as being central to the network or holding particular influence due to the dynamics of the food traffic, and stricter regulation in monitoring food trade here could benefit the network globally. Countries that take in many ingredients, process these into products, and act as distribution hubs are of particular concern.

"We found that the current structure of international food trade effectively makes The Netherlands a combined melting pot and Lazy Susan, with the busiest link to Germany," said Professor Baranyi. "This could explain why the tracing of the source suffered long delays in these countries in two serious outbreaks in 2011. This could be observed in both the E. coli outbreak in sprouts and the dioxin contamination in eggs."

The findings are supported by two types of analyses: one is based on the graph theoretical analysis of the structure of the international food trade network that allows the identification of the network core using the well- established "betweenness centrality" measures of nodes and edges for this purpose; the other is a measure based on the dynamics of the food-flow on the network, expressing to what extent a country is a "source" or a "sink".

Given the demonstrated complexity of the human food web, this work also introduces and validates for the first time a rigorous, quantitative methodology to help with biotracing and identifying the sources of food poisoning outbreaks, a problem that is only expected to increase in its magnitude, complexity and impact, in the face of current globalization trends.
